Thank you for joining the Heartbeat for Jesus 40-Day Plan! Our desire is to encourage you to grow stronger in your faith while moving your body through walking, jogging, or running.

Participation is completely voluntary.

By registering for this program, you acknowledge and agree to the following: Assumption of Risk

I understand that participating in the Heartbeat for Jesus 40-Day Challenge, including training plans, educational materials, emails, videos, optional community meetups, walks, runs, and

other related activities (collectively referred to as the program), involves physical activity and carries inherent risks.

These risks include, but are not limited to:

● Falls, slips, or trips

● Muscle strains and sprains

● Joint, bone, or soft tissue injuries

● Heat exhaustion or heat stroke

● Dehydration or overhydration

● Cardiovascular events

● Accidents involving vehicles, pedestrians, bicycles, or uneven terrain

● Other injuries, illnesses, or medical conditions that could result in serious injury, disability, or death

I voluntarily choose to participate in this Program and knowingly assume all risks associated with my participation.

Health Acknowledgment

I understand that Heartbeat for Jesus does not provide medical advice, diagnosis, or treatment.

I certify that I am responsible for determining whether I am physically able to participate in this Program. If I have any medical concerns, I understand it is my responsibility to consult my physician or other qualified healthcare provider before beginning or continuing participation. I agree to participate at a pace appropriate for my own fitness level and to stop exercising

immediately if I experience pain, dizziness, shortness of breath, illness, or any symptoms that make participation unsafe.

Release of Liability

In consideration of being allowed to participate in the Program, I, on behalf of myself, my heirs, executors, administrators, personal representatives, and assigns, hereby release, waive, discharge, and hold harmless Heartbeat for Jesus, Inc., its directors, officers, employees,

volunteers, team leaders, coaches, representatives, ministry partners, and affiliates from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses

arising out of or related to my participation in the Program, including participation in any optional community meetups, walks, runs, or volunteer-led activities.

This release includes claims arising from ordinary negligence to the fullest extent permitted under Florida law.

Participant Responsibility

I understand and agree that:

● Participation is voluntary.

● I am responsible for my own health, safety, and decisions while participating.

● I may stop participating at any time.

● I will follow all applicable traffic laws and exercise reasonable care while participating in

outdoor activities.

● I understand that participation in community meetups is optional and that I remain

responsible for my own actions and safety.

Photography & Media Release

I grant Heartbeat for Jesus permission to photograph, video record, or otherwise capture my participation during optional community meetups and related ministry activities. I understand these images or recordings may be used for ministry, educational, promotional, fundraising, website, email, social media, and marketing purposes without compensation.

Participant Certification

By checking the box during registration, I acknowledge that:

● I have carefully read this Agreement.

● I understand that this is a legal agreement.

● I understand that I am giving up certain legal rights by agreeing to this document.

● I voluntarily accept all terms of this Agreement.

Age Certification

I certify that I am at least 18 years of age.
